Q. Can I call my Health or Dental provider to change my address?
A. Yes, you can call your own and do a change of address. The numbers to call are listed below.
Q. Can I cancel or add a dependent to my insurance Med/Dental at any time?
A. Outside of Open Enrollment, changes are restricted. You can only make changes if you have a "Family Status Change." Family Status changes include marriage, divorce, death of child or spouse, birth or adoption of a child, or a change in your spouse's employment. A regular change from part-time to full-time or full-time to part-time shall allow for employees to change their benefit election. Temporary changes do not qualify as a change in status. If you have a qualified change in status and wish to make changes to your benefits, you have only 30 days from date of change to contact the HR department to make changes.
Q. Who is my dentist?
A. We do not keep record of your dentist here in the office but you can call PacifiCare Dental directly and by giving them your SS#, they can access your account and verify or make changes for you. They can also refer you to another dentist and/or new card.
Q. How much Personal Time Off do I have?
A. If you need to know your current PTO hours, please send a request in writing to PAYROLL and they will respond promptly to your request. Be aware that if it is Payroll time they will respond as soon as they are through with current payroll.
