Basic Employee Life Insurance:
The basic employee life insurance is provided to full-time and part-time employees at no cost to the employee. The coverage provided is a $20,000 benefit for full-time employees and $15,000 for part-time employees. The plan also includes Accidental Death and Dismemberment coverage under these circumstances.

Long Term Disability:
This benefit is provided to full-time and part-time employees at no cost to the employee. This plan covers eligible employees who work a minimum of 20 hours a week. In the event of a disability lasting more than 180 days, the plan will pay 60% of the employee's base salary, maximum of $6,000 per month until age 65. If the employee is 65 years old at the time of disability, the plan will pay according to a schedule.

Employee Assistance Program:
This benefit is provided through AffinityCare to full-time and part-time employees at no cost to the employee. All employees and their eligible dependents may use this service, which offers practical help for life's challenges. Employees and their family can call the toll free number 24 hours a day, 7 days a week for assistance. They can also receive up to three face to face visits with a licensed clinician per issue that arises in their life.
