| Options offers a comprehensive package of benefits for its employees. The benefit program is a Cafeteria plan that allows employees to make the choices that is most suitable for their individual needs. |
| Employees receive Flexible Credits, which may be used to pay for insurance benefits. Employees are eligible for benefits on the first day of the month following or coinciding with 30 consecutive days of employment. In addition, Options offers life insurance. long term disability insurance and an employee assistance program at no cost to the employee. |

Medical Insurance:
Employees may choose between Kaiser Permanente and Pacificare. Both plans offer extensive coverages ranging from routine office visits with $10 copayments, in-patient and out-patient hospitalization, maternity care, drug and alcohol care, and mental health programs. Both plans also offer prescription coverages with copayments of $5 or $10 for generic brands. |
Dental Insurance:
Employees are offered two different plans provided by PacifiCare Dental. The HMO plan offers routine office visits at a $5 co-pay. Also covered at no cost are oral exams, x-rays, cleanings and fillings. The PPO plan offers office visits, oral exams, x-rays and cleanings at no cost with the flexibilty of going to the dentist of choice. |
Supplemental Life Insurance:
In addition to the basic life insurance plan, employees who wish to purchase additional coverage for themselves or their family may purchase Supplemental Life Insurance. Employees may purchase up to an additional $100,000 of coverage for additional cost. Coverage for spouses can be purchased for up to $30,000. Life insurance for children may be purchased for a flat coverage amount of $10,000. |
Legal Assistance Plan:
The Legal Assistance Plan is offered through Pre-Paid Legal and payment for the program is on an after-tax basis. The plan allows employees and their family to receive legal services. The scope of services include unlimited telephone and office consultations, unlimited telephone calls made on behalf of the employee and family members, unlimited letters written by an attorney, and unlimited document review. |
AFLAC:
Employees are offered a choice of three different AFLAC plans. The Personal Short Term Disability plan offers a source of income if an employee becomes disabled due to an accident or illness that occur off the job. The Accident expense plan offers benefits to employees and their family in case of an accident. The Cancer Protector plan offers a range of benefits to employees and their family if they are diagnosed with cancer. |
Dependent Care Account:
The Dependent Care Account is a pre-tax spending account which allows employees to convert taxable salary dollars into tax-free benefit dollars to pay for child and dependent care expenses. Eligible expenses are limited to those allowed by law and subject to the maximum amounts allowed by the IRS. This plan has an irrevocable component and subject to forfeiture for monies deposited into the account and not spent during the plan year. |
| When employees purchase insurance coverage that exceed their Flexible Credits, the amount exceeding their credit is taken from payroll deductions. |
| Employees who do not need to obtain insurance coverage may utilize their Flexible Credits in either of these two choices: |
| 403(b) Tax Savings Account: This pre-tax retirement savings plan allows employees to set aside money up to the maximum allowed by the IRS. Employees make their own choices according to their own investment style, providing the employee with full control of the performance of their investments. |
| Cash: Employees who do not use part or all of their Flexible Credits will receive their credits in cash. The amount will be added to their base salary and will be subject to all federal, state and local taxes. |
